Immediate remote opportunity for RN's to work as a Transition Nurse for a company in Sacramento- Apply now Job Responsibilities: The Transition Nurse is responsible for identifying, overseeing and managing coordination for health plan members during changes or transitions between contracted programs and providers within an assigned region. Will be supporting the transition program (still part of Care Management). Will manage members who are needing post-discharge services from inpatient settings. The goal for the program is to mitigate re-admission to the hospitals. Will typically manage cases for 30 days. Will provide coordination for post discharge orders (i.e. home health, if anything was ordered in their discharge, connect with PCP appointments). Make sure members understand their discharge orders/provide them with the resources on what to do in certain situations. For example, they may be connecting members with complex case management or public programs that can continue their care. Qualifications/Requirements: Current, valid and unrestricted RN Licensure Required. Must have CA RN License. Graduate of an accredited nursing program. Bachelors Degree Preferred. Certified Case Manager preferred. Minimum three years clinical experience. Managed care experience required- including Case Management, Utilization Management, and/or Discharge planning. Experience working in a Managed Care Health Plan environment including Medicare/Medicaid. Experience with development of policies and procedures. Work Environment: Remote from Home (CA Only) Will be required to travel to Sacramento office for training's or office meetings. Full Time, M-F Schedule: 8AM-5PM Pay & Benefits: $40-45/hour based on experience level. Full Benefits included while on contract- medical, dental & vision.
